*It is recommended that each applicant saves their progress before submitting in the event any changes need to be made to application or to save any changes that can be used for future events.

Before beginning the form, please have these documents (if applicable) prepared to upload:

A Photo of your mobile food vendor setup
TSSA Field Approval OR a photo of the label affixed to the vehicle.
TSSA - MFSE Annual Inspection Report
Fire Suppression System Semi-Annual Report
