We appreciate your help in identifying street lights in need of maintenance. Simply complete the form below to submit a service request for street light maintenance.

We ask for your phone number in case City staff need to follow up with you for additional information.
Forms can be submitted 24/7; however, forms will only be checked during regular business hours.
If you provide an email address a copy of the form will be emailed to you for your records.
