2026 Arts Awards Nomination Form

Nomination Submission

Complete this online nomination by Monday, Feb. 9. Please include a detailed description outlining the outstanding contributions and background of the nominee. Award descriptions and eligibility details can be found at stcatharines.ca/ArtsAwards. Digital copies of support materials can be attached to the online form or emailed separately to culture@stcatharines.ca.

Important details:

1. The entire form must be completed on the same computer. You should be able to return to an incomplete form if using the same computer and the same browser.

2. Saving: You will need to save the page before moving to the next page. The form will automatically save and move to the next page after you click the continue button, located at the bottom of each page. If you need to step away or become distracted, the form will time out after approximately 10 minutes of inactivity. However, if you open the arts awards webpage again, you can use the link to take you to your partially completed form. 

3. You will receive a copy of the nomination via email once submitted.
   
   
4. To complete multiple nominations: After submitting your first nomination and receiving your copy of the nomination, simply return to stcatharines.ca/ArtsAwards and begin a new nomination form.
